Transfering EMTs or Adding KCSO as a Secondary EMS Affiliation
All EMTs in Search & Rescue need to list KC SAR as an affiliated agency with KC EMS in order to provide care at the EMT-B level.
Step 1:
Complete the Personal Status Change form (either online or a hard copy)

The KCSO SAR Coordinator will approve your status change online.

Print it out and send to the KCSO SAR Coordinator for approval and signatures.
Helpful info you'll need to complete WA DOH forms
Step 2:
Once you have received your WA state license, send a photocopy and a photocopy of your driver's license to the KCSO SAR Coordiantor.
Out of state EMT's:
Please fill out the application for reciprocity and mail a copy with your signature to the KCSO SAR Coordinator for approval and signatures.http://www.doh.wa.gov/hsqa/emstrauma/reciproc.htm
