Transfering active EMTs
Licensed EMTs (Transferring to KCSO EMS Agency or Adding KCSO EMS Agency Affiliation):
Step 1 - Complete the Personal Status Changes Form

(Kathy Decker will approve your status change online.)

Print it out and send to Kathy Decker for approval
Informaiton you will need info to complete your form:
EMS Agency #: 17R10 KING COUNTY SHERIFF'S OFFICE
Step 2 - Make a copy of your EMT License certificate (3x4) and CPR card and give to Kathy Decker.
Step 3 - Complete the KCSO/KCSARA EMT Sponsorship form and forward to Kathy Decker.
This form will activate the process to get your Online CBT access set up.
Step 4 - If using the paper vesion, have Kathy Decker sign the Personal Status Changes Form

Step 5 - Kathy will forward the Personal Status Changes form to KCEMS to obtain the Medical Program Director's signature.
Step 6 - Kathy will forward the KCSO/KCSARA EMT Sponsorship form to Joanne Bottenberg. Joanne will set up your Online CBT access and email you with your online password and login ID for http://www.emsonline.net
